Under Rowan's Law it is mandatory that all parents/guardians review the appropriate Concussion Resources with their child and acknowledge that they have done so. All coaches, trainers, managers, on-ice helpers and players over the age of 18 must also review the Concussion Resources and acknowledge that they have done so.
Participant Information
If you have more than one daughter registered, you will need to complete this form for each daughter. For team staff, you must complete this for yourself even if you have completed it for your daughter.

    Ontario Women's Hockey Association Rowan's Law Acknowledgement Form

    The Ontario Government has adopted the Statute “Rowan’s Law” that includes mandatory requirements for all sport, including the Ontario Women’s Hockey Association (OWHA). The Grand River Mustangs Girls Hockey Association is committed to concussion recognition and reporting. 

    The Concussion Awareness Resources and Code of Conduct must be reviewed on an annual basis.
